Vanderbilt University Medical Center (VUMC) is accepting application for the full-time Social Worker II - Psychiatric Consultant & Liaison Service position in our Transition Management Office (TMO) Transition Care, located on VUMC campus. This social worker role is dedicated to the psychiatric consult & liaison service and handles all psychiatric facility placements for VUH as well as behavioral health follow-up.
This is a key Social Worker role focused on admitted patients with behavioral health needs who require further psychiatric follow up. This social worker help transition patients to inpatient psychiatric facilities by completing thorough psychosocial assessments, gathering collateral data and following psychiatry team recommendations for next level of care. Based on the needs of the patients and current case load, this social worker also may provide bedside interventions as the patient is awaiting placement. This social worker often serves as a resource to other social workers regarding behavioral health interventions and resources in the community.
Maintains professional documentation which meets department and VUMC standards as it relates to compliance.
Provides brief targeted interventions for identified clients.
Completes psychosocial assessment and assists team with the development of the plan of care.
Provides education and or consultation to clients and families, treatment team and community.
Contributes to the organization by actively participating in process improvement initiatives and shared decision making.
